Камни мочеиспускательного канала и полости крайней плоти 12 pageПОЧКИ В почки часто метастазируют различные солидные опухоли. Поражаются почки также при лейкозах и лимфомах. По результатам 4413 аутопсий, метастазы солидных опухолей в почки (лейкозы и лимфомы были исключены) были выявлены в 18% случаев. Чаще всего встречались метастазы рака легкого (20%), молочной железы (12%), желудка (11%) и контралатеральной почки (9%). Как отмечено, метастатические опухоли часто врастают в почечную капсулу, но не поражают почечную лоханку. В 50% случаев наблюдаются метастазы в обе почки. Довольно часто наблюдаются альбуминурия и гематурия, изредка боль в пояснице и ХПН. Метастазы в почки характерны для злокачественных опухолей поздних стадий, наблюдаются, как правило, на фоне множественного поражения других органов, что свидетельствует о неблагоприятном прогнозе. Лечение зависит от локализации первичной опухоли. Так, при метастазах рака молочной железы или яичников лечение более эффективно, чем при метастазах рака легкого или желудка. Поданным аутопсий, при лимфогранулематозе и лимфомах почки поражаются одинаково часто — в 0,5—0,7% случаев. Метастазы при этом, как правило, двусторонние, множественные, в виде отдельных узлов. Метастазы лимфом в почки возникают, как правило, при диф- фузных крупноклеточных лимфомах одновременно с поражением других органов. Показана полихимиотерапия. В толще предстательной железы проходит проксимальная часть мочеиспускательного канала. Предстательная железа расположена в малом тазу и отделяется от лобкового симфиза позадилобковым пространством (пространством Ретциуса), а от прямой кишки — прямокишечно-пузырной перегородкой (фасцией или апоневрозом Денонвиллье). Основание предстательной железы прилежит к шейке мочевого пузыря, верхушка — к верхней поверхности мочеполовой диафрагмы. Лате-рально от предстательной железы проходит мышца, поднимающая задний проход. Предстательную железу кро-воснабжают ветви внутренней подвздошной артерии — нижние мочепузырные, внутренние половые и средние прямокишечные артерии. Венозная кровь оттекает в предстательное венозное сплетение, а из него во внутренние подвздошные вены. Предстательное венозное сплетение анастомозирует с глубокой дорсальной веной полового члена и мочепузырным венозным сплетением. Предстательная железа иннервируется ветвями нижнего подчревного сплетения. В норме поперечный размер предстательной железы на уровне основания 3—4 см, пе-реднезадний — 2—3 см, длина — 4—6 см. Дж. Мак-Нил предложил различать в предстательной железе периферическую, центральную и переходную зоны (рис. 23.1). На периферическую зону у молодых мужчин приходится 70%, на центральную — 25% и на переходную — 5% объема предстательной железы. Все зоны имеют отдельные предстательные проточки и, что гораздо важнее с кинической точки зрения, с разной частотой поражаются при доброкачественных и злокачественных опухолях. Рак предстательной железы в 60-70% случаев развивается в периферической зоне, в 10—20% случаев — в переходной и в 5—10% случаев — в центральной, в то время как аденома предстательной железы всегда возникает в переходной зоне (рис. 23.2). АДЕНОМА ПРЕДСТАТЕЛЬНОЙ ЖЕЛЕЗЫ Эпидемиология Аденома предстательной железы (доброкачественная гиперплазия предстательной железы) — самая частая доброкачественная опухоль у мужчин. Заболеваемость увеличивается с возрастом. Если на аутопсии в возрасте 41-50 лет аденому предстательной железы находят при- мерно в 20% случаев, то в возрасте 51—60 лет — уже в 50%, а в возрасте 80 лет и старше — в 90%. Хотя аденома предстательной железы с инфравезикальной обструкцией встречается реже, ее частота тоже увеличивается с возрастом: в возрасте 55 лет она составляет 25%, в возрасте 75 лет — 50%. Этиология Этиология аденомы предстательной железы изучена мало. По-видимому, это заболевание полигенное и гормонально-зависимое. Предстательная железа состоит из стромы и желез, и каждый из этих компонентов может подвергаться гиперплазии и вызывать инфравезикаль-ную обструкцию. Существуют препараты, применяющиеся при аденоме предстательной железы и избирательно действующие на железистую ткань и строму органа. Факторы риска аденомы предстательной железы не известны. Одни авторы отмечают наследственную предрасположенность, другие — расовые различия в заболеваемости. Вполне возможно, что примерно у половины больных моложе 60 лет, которые нуждаются в хирургическом лечении, заболевание наследственное. Тип наследования, скорее всего, аутосомно-доминант-ный, поскольку наличие родственника первой степени, больного аденомой предстательной железы, повышает ее риск в 4 раза. Экспериментальные и клинические исследования выявили два фактора, необходимые для развития аденомы предстательной железы, — дигидротестостерон и старение. Эксперименты на собаках показали, что с возрастом чувствительность предстательной железы к андрогенам повышается. Любопытно, что возрастные изменения в предстательной железе у собак вызваны не столько ускоренной пролиферацией клеток, сколько их замедленной гибелью. В результате этих наблюдений было выдвинуто несколько теорий патогенеза аденомы предстательной железы. Согласно первой из них, в развитии опухоли играет роль взаимодействие клеток стромы и эпителиальных клеток. Не исключено, что клетки стромы обеспечивают ауто- и паракринную регуляцию, вырабатывая фактор роста фибробластов или трансформирующий фактора роста (3. Вторая теория патогенеза предполагает, что с возрастом нарушается апоптоз клеток предстательной железы. Возрастные изменения предстательной железы у животных объясняют действием эстрогенов, синергичным с действием дигидротестостерона. У собак эстрогены стимулируют синтез андрогеновых рецепторов, меняют метаболизм стероидных гормонов, повышая уровень дигидротестостерона в ткани предстательной железы, стимулируют продукцию коллагена в строме и в присутствии андрогенов предотвращают гибель клеток. Клинические наблюдения свидетельствуют о том, что аденома предстательной железы — заболевание гормонально-зависимое. После двусторонней орхиэктомии Глава 23. Опухоли предстательной железы
