Риск прогрессирования рака предстательной железы после I хирургического лечения зависит от степени дифферен-1 цировки, морфологической стадии и обширности прорастания капсулы предстательной железы. Риск прогрессирования выше при наличии опухолевых клстокв крае резекции, выходе опухоли за пределы капсулы предстательной железы, прорастании в семенные пузырьки и при низкодифференцированном раке. Отличить рецидив рака предстательной железы отдиссеми- | нации при повышении уровня ПСА после радикально! j простатэктомии можно по сроку от операции до повышения уровня ПСА, времени удвоения уровня ПСА, а также с помощью методов лучевой диагностики и биопсии ложа удаленной предстательной железы. Диссе-минацию следует заподозрить, если ПСА в сыворотке обнаруживается сразу после операции либо вскоре после нее, а также при быстром удвоении уровня ПСА. Если же ПСА появляется в сыворотке через долгий срок после операции или его уровень удваивается медленно, то у больного, скорее всего, рецидив. Это подтверждается результатами исследований. Показано, что после хирургического лечения ПСА в сыворотке при рецидиве появляется позже, чем при диссеминации (через 33 и 20 мес соответственно). Удвоение уровня ПСА в первые 6 мес после радикальной простатэктомии характерно для диссеминации. В одном из исследований приняли участие 114 больных после радикальной простатэктомии, у которых в сыворотке определялся ПСА. Диссеминации не было ни у одного больного, в то время каку 53% был обнаружен рецидив. Часто для его диагностики требовалась многократная биопсия ложа удаленной предстательной железы. После первой биопсии диагноз был установлен у 67% больных, после второй — еще у 18%, после третьей — еще у 10% и, наконец, после4-й-у оставшихся 5% больных. При рецидиве рака предстательной железы после хирургического лечения в отсутствие регионарных и отдаленных метастазов рекомендуется лучевая терапия. Прогрессирование после лучевой терапии. Повышение уровня ПСА после лучевой терапии также свидетельствует о прогрессировании. При рецидивах возможны паллиативная простатэктомия или криодеструкция, однако при этом часто встречаются осложнения и высока вероятность дальнейшего прогрессирования. Диссеминированный рак предстательной железы Гормональная терапия. Поскольку большинство больных раком предстательной железы умирают из-за дис-семинации, активно разрабатывается лечение диссе-минированного рака предстательной железы. Хорошо известно, что в большинстве случаев рак предстательной железы — гормонально-зависимая опухоль и разные схемы антиандрогенной терапии эффективны у 70—80% больных с диссеминацией. Примерно 95% тестостерона — основного циркулирующего андрогена — вырабатывается в яичках клетками Лейдига, небольшая его часть синтезируется вне яичек из других стероидных гормонов. Примерно 98% тестостерона связано с белками плазмы. В клетки предстательной железы проникает только свободный тестостерон, здесь он превращается в основной внутриклеточный андроген — дигидротесто-стерон. Дигидротестостерон связывается с внутриклеточным белковым рецептором, а затем в связанном виде проникает в ядро, где регулирует транскрипцию. Действие андрогенов можно блокировать на разных уровнях гипоталамо-гипофизарно-гонадной системы с помощью разных препаратов (табл. 23.9). Использование аналогов гонадолиберина длительного действия, которые вводятся с интервалом 1 или 3 мес, позволяет избежать орхиэктомии и применения диэтилстильбэстрола. Самыми частыми методами антиандрогенной терапии в настоящее время являются применение аналогов гонадолиберина и орхиэктомии. При метастазах в позвоночник со сдавлением спинного мозга и у больных с ДВС-синдромом предпочтителен кетоконазол, который отличается быстрым действием. Хотя тестостерон является основным циркулирующим андрогеном, в надпочечниках синтезируются и другие андрогены — де-гидроэпиандростерон, дегидроэпиандростерона сульфат и андростендион. Ряд авторов считают, что устранение андрогенов яичкового и надпочечникового происхождения (полная андрогенная блокада) дает более быстрый и длительный эффект, чем блокирование секреции только яичковых андрогенов. Полная андрогенная блокада подразумевает применение антиандрогена в комбинации с аналогом гонадолиберина или орхиэктомией. Антиандрогены блокируют андрогеновые рецепторы, с которыми связывается дигидротестостерон — внутриклеточный андроген, участвующий в пролиферации и дифференцировке клеток предстательной железы. При сравнении групп больных, стандартизованных по дис-семинации и общему состоянию, после полной андро-генной блокады выживаемость оказалась выше, чем после применения только аналогов гонадолиберина. Однако в другом исследовании эффективность анти- андрогена и комбинации антиандрогена с орхиэктомией оказалась одинаковой. Сейчас изучается эффективность периодической андрогенной блокады. Исследователи надеются, что эта методика замедлит развитие резистентности опухоли к гормональной терапии. Долгие годы широко обсуждается вопрос о том, когда следует начинать гормональную терапию при раке предстательной железы. По результатам кооперированного исследования, проведенного в 1960-х гг., ранняя гормональная терапия не увеличивает выживаемость при дис-семинированном раке предстательной железы. Однако в контролируемом исследовании, проведенном Британским Советом медицинских исследований, оказалось, что чем раньше начата гормональная терапия, тем выше выживаемость и ниже частота осложнений (сдавление спинного мозга, обструкция мочеточника, инфравези-кальная обструкция, патологические переломы). Неэффективность гормональной терапии. Больным, у которых на фоне полной андрогенной блокады повышается уровень ПСА, рекомендуется отменить антиандроген. Примерно в 20—30% случаев уровень ПСА снижается и отмечается уменьшение размеров опухоли. Почему наступает этот эффект, который называют синдромом отмены антиандрогенов, не известно. Существует мнение, что резистентность опухоли к гормональной терапии возникает из-за изменений андрогеновых рецепторов. Антиандрогены блокируют андрогеновые рецепторы, но вполне вероятно, что измененные рецепторы они стимулируют. Таким образом, после прекращения стимуляции (отмены антиандрогена) и возникает ремиссия.
