равен 0%, при дозе 25—35 Гр — 2%, при дозе 40—45 Гр — 6%. По одним данным, впервые 10 лет после лучевой терапии хотя и немного, но повышается риск вторичных лейкозов и солидных опухолей, преимущественно мочевого пузыря, легких и ЖКТ. Другие авторы эти данные не подтверждают. Согласно более поздним публикациям, относительный риск злокачественных опухолей легких и мочевого пузыря составляет 2,5. Не исключено, что риск опухолей ЖКТ тоже повышен, но это доказано не столь убедительно. Рак почки, почечной лоханки и мочеточника Основной метод лечения рака почки — хирургический. В двух контролируемых клинических испытаниях адъ-ювантная лучевая терапия не повышала выживаемость. Вероятно, лучевая терапия целесообразна при неоперабельных опухолях, высоком анестезиологическом риске, а также при наличии опухолевых клеток по краю резекции. Результаты нескольких клинических испытаний предполагают, что лучевая терапия при раке почки эффективна, особенно при низкодифференцированных опухолях, прорастании в околопочечную клетчатку и метастазах в лимфоузлы. Судя по ретроспективным данным, нельзя исключить, что адъювантная лучевая терапия уменьшает риск рецидива у больных раком почечной лоханки или мочеточника поздних стадий (ТЗ—4), а также при метастазах в лимфоузлы. Рак мочеиспускательного канала у женщин При раке мочеиспускательного канала у женщин и дистанционная, и контактная лучевая терапия так же эффективны, как хирургическое лечение. Для излечения опухолей ранних стадий с поражением дистальной части мочеиспускательного канала достаточно только контактной или сочетанной лучевой терапии. Частота полных ремиссий достигает 80—90% при 5-летней общей выживаемости более 65%. При более крупных или прорастающих в соседние структуры опухолях отдельные авторы рекомендуют перед внутриполостной лучевой терапией проводить дистанционную с профилактическим облучением паховых, наружных и внутренних подвздошных лимфоузлов. Схема внутриполостной лучевой терапии при раке мочеиспускательного канала у женщин представлены на рис. 28.2. Опухоли, занимающие весь мочеиспускательный канал или его проксимальную часть, часто прорастают в мочевой пузырь и метастазируют в лимфоузлы. В этом случае показаны неоадъювантная лучевая терапия и тот или иной вариант экзентерации малого таза. Опухоли поздних стадий удается удалить только у 20—30% больных, поэтому 5-летняя общая выживаемость невысока. После комбинированного лечения (хирургического и лучевой терапии) 5-летняя общая выживаемость чуть выше. Единственный шанс на излечение дает агрессивное комбинированное лечение. Так, по данным некоторых авторов, при опухолях диаметром более 4 см 5 лет и более живут только те больные, которым выполнена экзентерация малого таза и проведена послеоперационная лучевая терапия. Обнадеживающие результаты дает комбинированное лечение с включением неоадъювантной лучевой терапии. Осложнения лучевой терапии наблюдаются часто, но, как правило, они преходящие. Чаще всего возникают стриктуры мочеиспускательного канала, реже недержание мочи, лучевой цистит и стеноз влагалища, еще реже свищи и тонкокишечная непроходимость (они могут быть также обусловлены не только лучевой терапией, но и самой опухолью). Рак мочеиспускательного канала у мужчин и рак полового члена Основной метод лечения рака полового члена — хирургический. Несмотря на его высокую эффективность, у многих больных (особенно с нормальной потенцией) стараются избежать калечащих операций. Завершенных контролируемых клинических испытаний, сравнивающих хирургическое лечение с лучевой терапией, пока нет. Поскольку лучевая терапия является эффективным органосохраняющим методом лечения, ряд авторов рекомендуют назначать при раке полового члена ранних стадий именно лучевую терапию, а при рецидивах прибегать к операции. При опухолях диаметром более 4 см показано хирургическое лечение. При небольших злокачественных опухолях и предраковых заболеваниях эффективны ортовольтная рентгенотерапия и (3-терапия с использованием пучков электронов средней или низкой энергии, которые поглощаются преимущественно поверхностными тканями. Контактную лучевую терапию применяют при обширном поверхностном поражении полового члена. При крупных опухолях тела полового члена и дистальной части мочеиспускательного канала и глубокой инвазии проводят сочетанную лучевую терапию — внутритканевую с последующей дистанционной. Логично предположить, что при опухолях поздних стадий и низкой степени дифференцировки целесообразно профилактическое облучение регионарных лимфоузлов, однако эффективность этого не доказана. Облучение в суммарной очаговой дозе 50—60 Гр позволяет добиться полной ремиссии у 80—100% больных раком полового члена ранних стадий и у 60% больных раком полового члена поздних стадий. При опухолях ранних стадий на регионарные лимфоузлы можно не воздействовать, ограничиваясь наблюдением. Их удаляют только при возникновении метастазов. Существует и другая тактика — профилактическая дистанционная лучевая терапия на зоны регионарного метастазирова-ния. Она эффективна в 95% случаев. Без профилактической лучевой терапии метастазы в лимфоузлы возникают у 20% больных. Лучевая терапия также показана при I Глава 28. наличии пальпируемых паховых лимфоузлов у больных, которым выполнено вмешательство на первичной опухоли. Показано, что это статистически значимо снижает риск прогрессирования. При раке мочеиспускательного канала у мужчин, как правило, проводят хирургическое лечение. Однако при поражении его подвижного отдела возможна также лучевая терапия. Первичный рак подвижного отдела мочеиспускательного канала у мужчин лечат так же, как у женщин, рак предстательной части мочеиспускательного канала — как рак предстательной железы. При раке мочеиспускательного канала высок риск прогрессирования. Пятилетняя общая выживаемость при поражении подвижного отдела составляет 55%, фиксированного — только 15%. Предполагается, что при раке мочеиспускательного канала поздних стадий можно проводить не-оадъювантную химиотерапию и лучевую терапию, а при их неэффективности или рецидиве прибегать к хирургическому лечению. Эта методика проходит сейчас I и II фазы клинических испытаний, проводимых Онкологической исследовательской группой Юго-Запада США. Самое частое (вплоть до 50% случаев) позднее осложнение лучевой терапии по поводу рака мочеиспускательного канала у мужчин и рака полового члена — стриктура мочеиспускательного канала. При ее возникновении проводят бужирование. Риск стриктуры мочеиспускательного канала зависит от методики лучевой терапии и суммарной очаговой дозы. Изредка наблюдается некроз мягких тканей. Потенция после лучевой терапии сохраняется почти у 90% больных. ЗАКЛЮЧЕНИЕ Табл. 28.13 суммирует роль дистанционной лучевой терапии в онкоурологии. Тесный контакт уролога с луче- вым терапевтом позволяет планировать эффективное лечение при минимальном риске осложнений.
