Камни мочеиспускательного канала и полости крайней плоти 1 pageВ мочеиспускательный канал камни чаще всего попала-1 ют из мочевого пузыря, реже из верхних мочевых путей. I Большинство камней мочеточника, прошедших в моче-1 вой пузырь, отходит беспрепятственно через мочеис-1 пускательный канал. Камни мочеиспускательного ка- j нала образуются при застое мочи в дивертикулах, в \ области стриктур и рубцов после перенесенных ранее { операций. Большинство камней мочеиспускательного канала у мужчин одиночные, расположены в предстательной или луковичной части мочеиспускательного I канала. При рецидивирующих камнях подвижного от-1 дела мочеиспускательного канала в отсутствие другой I органической патологии следует подозревать, что боль-1 ной вводит их в мочеиспускательный канал сам. Это на-1 блюдается при синдроме Мюнхгаузена и наркомании. УI женщин камни мочеиспускательного канала наблюла-1 ются редко и образуются, как правило, в дивертикулах, | Это связано с тем, что у женщин мочеиспускательный I канал короче, а камни мочевого пузыря возникают ре-1 же, чем у мужчин. Клинические проявления камней мочеиспускатель-1 ного канала те же, что и камней мочевого пузыря — пре-рывистое мочеиспускание, терминальная гематурия к I инфекции. Возможны острая задержка мочи, парадок-1 сальная ишурия и сильная боль, иррадиирующая у муж-1 чин в головку полового члена. Диагноз подтверждаю! I при пальпации, уретроскопии или уретрографии. Тактика лечения зависит от основного заболевания. При камнях на фоне выраженной стриктуры или дивер-1 тикула мочеиспускательного канала выполняют откры-1 тое вмешательство — пластику мочеиспускательного канала или иссечение дивертикула. При длительно не смещающихся камнях мочеточника приходится выполнять уретротомию. Камни полости крайней плоти, или препуциальные камни, встречаются редко. Как правило, они возникают у взрослых на фоне выраженного фимоза. Другая причина — несоблюдение личной гигиены. Диагноз устанавливают при пальпации. Показано круговое обрезание или рассечение крайней плоти по дорсальной поверхности. Эти операции предупреждают повторное образование камней. Идея использования ударных волн для дробления камней появилась в 1950-х гг. в России. Специалисты немецкой авиационной компании Dornier во время испытаний сверхзвукового самолета столкнулись с тем, что при движении различных объектов в атмосфере могут возникать ударные волны, разрушающие твердые тела. Неожиданным прикладным применением установки, разработанной для моделирования этих волн, и стала дистанционная литотрипсия. Успешное дробление мочевых камней в клинике было впервые проведено в 1980 г. на базовой модели дистанционного литотриптора компании Dornier HM-1. В1982 г. появилась более совершенная модель НМ-2, а в 1983г. — НМ-3 (рис. 18.1). Последняя получила широкое распространение в Европе, Японии и США, в декабре 1984 г. была одобрена FDA. Появление дистанционной литотрипсии изменило тактику лечения мочекаменной болезни. Во всем мире сейчас функционируют тысячи дистанционных литотрипторов, с помощью которых успешно излечены миллионы больных. В дальнейшем компания Dormer усовершенствовала параболические рефлекторы и компьютеризировала движение терапевтической головки аппарата. В настоящее время дистанционные литотрипторы выпускают разные компании. Дистанционный литотриптор состоит из источника волн, среды воздействия (для передачи энергии внутрь тела) и системы визуализации (рентгеновской установки, ультразвукового аппарата или и того, и другого). Дистанционные литотрипторы, выпускаемые разными компаниями, различаются по интенсивности боли во время лечения, виду анестезии, возможности применения при желчнокаменной болезни и контрактурах суставов, пиковому давлению в зоне терапевтического воздействия (400—1500 атм), фокусу (от 6х28до50х 15 мм), фокусному расстоянию (расстоянию между источником ударной волны и точкой схождения волн, 12—17 см), размерам, мобильности, конструкции модулей, цене, стоимости эксплуатации и расходных материалов, а также по сроку службы. СВОЙСТВА УДАРНЫХ ВОЛН В отличие от ультразвуковой волны, параметры которой меняются синусоидально, ударная волна представляет собой резкий скачок давления, приводящий к появлению сил сжатия (рис. 18.2). Существуют два основных типа источников ударных волн — электроразрядные и конечно-амплитудные. Электроразрядные источники ударных волн Образование и расширение плазмы при электрическом разряде приводит к возникновению ударной волны. Аналогичное явление наблюдается во время грозы: за молнией (электрическим разрядом) следует гром (звуковой удар). Свойства ударных волн интенсивно изучались во время подводных взрывов. Сразу после образования скорость ударной волны превышает скорость звука в воде, затем быстро сравнивается с ней. По мере движения ударной волны быстро и нелинейно падает давление. Концентрируя ударную волну в одной точке, можно с успехом дробить камни (рис. 18.3). Фокусировка достигается при использовании рефлекторов, форма которых в разрезе близка к эллипсу.
